if you lift the seating at the saloon vent you will see a 90mm duct connected to a three way junction one end goes straight to the vent with a short bit of hose the other end is reduced down to 50mm which then carrys on to the front heads where a tee piece is fitted one end to the vent in the heads the other to the cabin. There must be three metres of 50mm duct. WHY would the warm air squeeze down this small duct when it can just gush out unrestricted into the saloon.
THE QUICK FIX
remove the three way junction and reassemble so the flow goes forward [if the air wants to escape to the saloon make it work for it]
remove the tee piece and turn it so the through section goes straight to the heads vent; i found i could loose about one metre of 50mm duct. connect the cabin duct to the tee. I now have usefull warm air at either vent but not so good if both are open together. will see how it goes
THE TRUE FIX
run 90mm duct all the way through keeping 50mm branches as short as possible. DAVID

My heater is a Webasto and I am recalling from memory that the ducts are 100 mm and 60 mm respectively. I completely agree with you, there is no point letting the last duct going to the forepeak be narrower. I will probably change it to 100 mm in the future.

i had a similar problem and fixed it by replacing the open/louvered vent in the salon (main duct) with a closable one.  This means I can direct all the heat forward by closing everything but the forepeak duct.
 
It might not be what webasto etc like, but on a cold night who cares, we did this and after heating the forepeak, opened the main one a little to let the main duct "breath".
 
It is important to let the main duct in the saloon breathe and leaving it closed al night would not be advisable.
